Documents for Doctors
Centrelink removed their medical report form from the DSP application in 2015, and now rely on “raw” medical evidence. At Disability Support Pension Australia Applications and Appeals, we have created an easy to fill out medical form for doctors to help address all the necessary criteria for medical eligibility. This should be used in conjunction with the relevant “Tables” along with the other guides provided here. This will ensure the best possible results for your application.
These forms are accepted by Centrelink and have been very successful for a great number of people already, for original claims and for appeals.
This form is ideal for specialists and/or Gp’s to provide details of each medical condition and how it impacts your ability to function.
The form can be filled out digitally if preferred. Alternatively doctors can use this form as a template as a basis for their own report.
The Tables
The previously provided Tables legislation has been broken up into two sections here. The first link is a shortened version of the rules on how to use the tables to allocate points. It is a good idea to be familiar with this and to have a copy on hand for doctors just in case they require it.
The second link is where you can download the individual tables. You will need a copy of each table relevant to your conditions to provide to your doctors. Ideally highlighting the rating that you believe applies to your symptoms. Use as many tables as are relevant.
